 Guidelines for completion of application form for the Maintained Schools sector

 

The form is designed for initial use by the school applying for registration, with subsequent completion by the consultant after the visit has been made.  Please use these guidelines to assist with completing the relevant parts of the form. 

Please do not exceed a maximum of 10 pages when completing this form.

Schools are required to complete the form digitally and return to the Administrator by email, with a signed hard copy, the completed checklist and requested documentation returned by post.

Please note that it is essential for the cost-effective operation of visits that all points are responded to in full.  For example, please do not answer ‘Refer to prospectus’.   By filling in the form digitally, schools can ‘cut and paste’ from their own documentation.  Full information will enable CReSTeD to keep costs down for the schools.  Forms that have not been completed adequately will be returned to schools.

Schools are required to send all requested documents at least 21 days in advance of the visit.  CReSTeD wishes to assure schools that all information received will be treated in complete confidence by CReSTeD and the consultants contracted to carry out the visit will adhere strictly to Data Protection legislation.

The form is set up so that responses may be made in the grey sections which will expand according to the amount of information you enter.  The grey colour will not show up on the printed document.  Points in italics are for completion by CReSTeD consultants.  Consultants are not obliged to make comments, but space is available if required.

 

At the beginning of the form, please indicate whether you are applying for a new registration or re-registration.

Accepted age range – this refers to the age range accepted in the school, not the actual age range at the time of application.

Consultants should enquire about the turnover of pupils at the school, and if this appears high, the reasons.

 

e.

This refers to the whole school, not just with regard to dyslexic pupils.

 

f.

Please give details of type of buildings, sports facilities, acreage etc.

Consultants:  Please comment on maintenance standards, etc.

 

g.

Include house or pastoral system structure and principles, eg individual focus on each child

Consultants:  Please meet with Head of Care.

For completion by consultants only:  please comment on each document.  Please look in general at website content.

6.

a.

The school should enter details of qualifications of learning support staff.  Include the Awarding body.   The school should include details of on-going training. Please also include details of qualifications of the SpLD specialist the school has access to, who may not be a staff member. Please complete carefully as this item is frequently completed incorrectly, presenting problems and delays in registration.  However please just list qualifications (not abbreviated CVs!). Consultants should ask to see at least some certificates (originals, not copies).

 

d.

Consultants should particularly look for evidence of relevant Continuing Professional Development and the outcomes that impact on the provision for dyslexic pupils.

b.

Please give details of school policy or practice regarding accessing the National Curriculum.  Consultants: please use Lesson Check List.

9.

a.

Please explain all methods you use, eg screening, teachers assessment, information from parents, classroom observation, and how you ensure all information is captured and used.

10.

a.

Please explain how these are monitored.

 

b.

Please enclose two examples.

 

c.

Include details of tests used and monitoring procedures.

 

d.

Please send two brief case studies of dyslexic children in the school showing how:

  • they were identified
  • information from assessment was included on the IEP
  • support strategies were put in place for curriculum access
  • literacy support was provided
  • monitoring was acted upon.

 

e.

In the Key Stage 2 & 3 sections, A/D = percentage absent or disapplied.  Please give any other information that may be relevant, eg Common Entrance, GNVQ etc.

 

f.

Give details relating to internal and external examinations.

 

h.

Consultants – Ask at least three pupils, of different year groups.

13.

Schools must provide a list of parents of dyslexic pupils: 3 from each year group. Ten names will be selected at random by the Consultant who will ask for contact telephone numbers prior to the visit.  Six of these parents will be contacted before the visit takes place.  A letter from CReSTeD explaining the visit process is available to send to parents whose agreement should be obtained prior to providing their telephone numbers.  Consultants: please contact six parents and comment on their responses.  Further calls, to the same or different parents, may be made after the visit if required.
